It depends mainly on where you are in the current situation which is the same as how I got Steppin' Razor.

No, it's a Multiplayer Achievement. Triple Kill with Energy Sword in Lone Wolves. It's only possible to get if you are doing Swords or are on the maps Construct or The Pit.
